Here are the two forms that need to be submitted, along with a digital photo headshot.

• CFSL Liability waiver
• FSSA Liability waiver

Payment

This allows you to pay for your league fees using a credit card via Paypal.
A jersey fee for new players will be handled by your team captain.
Please do not pay until you have submitted all forms and photos, and have received email confirmation of league and team enrollment.
